45-day refill rule
For 7.5 mg, 10 mg, 12.5 mg and 15 mg, Lilly’s Zepbound Journey Program price of $449/month applies when you refill within 45 days of the prior delivery/receipt. Miss that window and the regular price applies: $499 (7.5 mg) or $699 (10–15 mg). That can be a $250 swing in one month at higher doses.
Use our free refill reminder tool if you are tracking a 45-day window.
Vial vs KwikPen
- Self-pay dollar amounts by dose are the same for vial and KwikPen on Lilly’s published structure.
- Vials: home delivery or retail pickup via LillyDirect; inject with separate needle/syringe.
- KwikPen: multi-dose pen; may also use a pharmacy savings card path described on Lilly’s savings pages.
- Medicare GLP-1 Bridge covers Zepbound KwikPen only, not vials — see Medicare Bridge.

Note: LillyDirect pages intermittently error for automated fetches; figures above match Lilly’s published self-pay footnotes reported on LillyDirect (2.5 mg $299; 5 mg $399; 7.5–15 mg $449 within 45 days, else $499/$699) and our prior Oct 31, 2026 check.
FAQ
How much is a Zepbound vial without insurance?
On LillyDirect self-pay, $299/month for 2.5 mg, $399 for 5 mg, and $449 for 7.5–15 mg when each refill is within 45 days; otherwise $499 (7.5 mg) or $699 (10–15 mg). A month is 28 days (4 vials).
Is the vial price the same as the KwikPen?
Lilly’s published self-pay structure uses the same dollar amounts for vial and KwikPen at each dose. Needles/syringes are a separate purchase for vials.
Does Medicare Bridge cover Zepbound vials?
No. Medicare says the GLP-1 Bridge covers Zepbound KwikPen only—not single-dose vials or pens.
What is the 45-day rule?
For 7.5–15 mg, the $449 Journey Program price requires refilling within 45 days of the prior delivery/receipt; late refills pay the higher regular price.
